My fee for a 53-minute individual therapy session is $140. For a couples or family session, my fee is $160. Consultation meetings with parents are charged at the rate of $140. Payments are to be made at the beginning of each session. Cash, check, and credit are acceptable forms of payment.
As a Licensed Therapist (Lic# LH60525325), I am insurance eligible for all insurance carriers in Washington State. I am currently an approved provider for Premera (includes many out of state Blue Cross/Blue Shield plans), Cigna, and First Choice Health (includes Kaiser Permanente PPO) and will submit claims accordingly. Otherwise, I do not file insurance claims for you or receive direct insurance payments, except for Premera, Cigna, and First Choice Health. You may receive out-of-network reimbursement from other insurers. The specific amount of this reimbursement depends on your plan, and your insurer will be able to tell you exactly what this reimbursement would be. If you wish to use your insurance, you must arrange for the carrier to reimburse you directly. You are responsible for obtaining and filling out any appropriate paperwork and submitting it to your carrier, as well as knowing the benefits and limits of your policy. To assist in obtaining reimbursement for out-of-network services, I will be glad to provide information that the insurance company deems necessary. Please note that use of insurance requires a mental health diagnosis.
You may cancel an appointment with at least 48 hours notice. If you have not given 48 hours notice, you‘ll be responsible for paying for your reserved time slot. Of course, insurance companies will not cover missed or late appointments. This payment will be due at our next meeting. Please note that in the case of a late arrival, appointments will still end at their scheduled time.
